There are too many instances of this in Sierra to list them all.
Most annoying is the one when you close Finder, VO will repeat whatever
its focused on 3 times. Closing any other app does not do this.
Also at login I hear "Authentication has new window" twice.

On my Macs VO became much more expressive since Sierra. On the Desktop
it repeats „Macintosh HD“ quite often several times after navigating to
Desktop. If I open my MacBook Pro VO keeps saying „One system dialogue
displayed“ (not exactly because translated from german). After 3 times
repeating this it stops but not always. So, you are not alone with this
issue. ;-)

Mine just says it once. Perhaps you have a few items in queue to be
downloaded? Turn off the network and then look at the downloads area
in Safari and clean it up if necessary.
