There are two settings for waking and speaking. First, you can choose between waking when you tap the screen, and waking when you raise your wrist. To be clear, you can have one, the other, or both. Then you choose whether you want VO to speak on wrist raise. I don't, since there are enough false positives throughout the day that VO would be chattering too much for me.
This means that you can have it speak when you raise your wrist, or have it remain silent until you touch the screen. It will always speak when you tap the screen When it speaks, if there's a notification, it will read the app name (Messages, Twitter, etc) first. You swipe right, or touch elsewhere on the screen, to learn more about the notification. How the text is broken up is up to the app developer.
