tried it with my iPhone. So, I just did, and it worked perfectly,
including the ability to record in QuickTime.
1. Connect the iPhone to the Mac with the USB charging cable.
2. Open QuickTime.
3. Unlock your iPhone. I'm not sure if this is necessary, but I do it
anyway.
4. Up to the File menu and down to "New Movie Recording", or press
cmd-option-n.
5. In the Show Captured Devices menu, select your iPhone. It should
normally auto-select the iPhone for audio as well, but I'd confirm just
in case.
6. VO-left to the Full volume button.
7. Swipe around the iPhone screen and you should hear your iPhone audio
coming through the Mac. The video probably is too, but we just can't
see that part :).
8. On the Mac, VO-right to the Start Recording button and a QT movie
will be recorded until you press the Stop button.
9. Press the Play button to watch your handiwork and save it if you're
satisfied.
I'm not sure what could be happening for you. I use this method often
when doing presentations so that my iPad screen is projected.

Two things. First, to address the Photos thing. When in the Photos
app on your Mac, you can interact with the Navigation bar in the
Photos screen and uncheck the box for that prompt. Second, check
that your iPhone is visible when in iTunes. I believe that you need
to have trusted the interaction between the devices.

This is totally doable on iOS if you also have a Mac. You can
connect the iOS device to your Mac with the USB charging cable. Go
to QuickTime on your Mac and choose "New Movie Recording" from the
file menu. In the resulting window, VO-space on the "Show Capture
Devices" pop-up and select the iPad or iPhone for both video and
audio capturing. By default, the volume seems to always be muted,
so VO-left to the Full Volume button and press it. Finally,
VO-right to the Start Recording button and press it. Because you are
doing a movie recording, you get both the video and audio from the
iOS device recorded by QuickTime. Press the Stop button to end the
recording and edit/trim/save your product.
This appears to be more challenging on the Mac. I can use an app
called Reflector to capture the video content, but the VoiceOver
Audio is more challenging. I've got some ideas, but need to round
up a cable or two to test before I make any commitments as to its
usability. The audio and video is mirrored when using Reflector and
an iOS device, but I haven't bothered messing with recording this
way yet.
