This should not be so difficult. Everything I found in Google
searches says 'save' from the markup dialog but there is no such thing.
What I found is in the Share dialog where I tried Preview. That gave me
the save option.
I did find what you need when using cmd-shift-4. Press the space
bar. That turns the cursor into a camera. Move it to the window you want
and click.
Not really intuitive.

cmd-shift-4, used to just take a screen shot and place it on the
clipboard, but that doesn't seem to work for me. One thing to note
though, if you take a screen shot with cmd-shift-f3, the Markup layer
gains focus and unless you click the physical mouse, it seems to cause
weird things to happen such as cmd-w not working, cmd-shift-5 and a few
others. As I mentioned, clicking the physical mouse brings focus back
into the real world and cmd-w and such work again. So, see if the click
helps you.
