Personal preference. I set it to on which allows me to wrap on menus
for one thing.

As ET mentioned, the cursor tracking items are in the VO Utility under
the Navigation category. The items which has VO announce what's under
the mouse, is in the Verbosity category, in the Announcements tab.
You can toggle this on/off and set the delay before announcing. I
have it on, and no delay at all.

I tend to havel cursors synchronized, and also have the mouse
follow VO focus. I've also set VO to read what's under the mouse
after a 0 second delay. It works well for me except in some web
situations whereby if the mouse is on certain locations, VO has
focus difficulties. Very rare though. So, overall, it's kind of
a personal preference.

control-option-arrows moves the vo cursor. It can go where
keyboard focus might not can. Arrows or tab moves the system
keyboard focus.
It is usually not advisable to have the mouse follow vo, but if
you lock the mouse it will follow vo automatically.
