Several misc. ideas below. Perhaps one will solve issue. It sounds like your command key is not responding correctly. Try turning on keyboard help with VO-k and press keys to make sure you know what is where. Also, make sure vo lock is not on by pressing the semi-colan. (next to the "l" key) . Just to be sure the command key is to either side of the space key. Did you experiment with voice command or dictation? I think one of these can use a command key turning off its standard behavior. One last idea, there is a reset VoiceOver to default in (I believe) the file menu, if you think a change done was while you were in VO utility this could be worth trying.
