Hi,

Well, if VoiceOver ever said Speech off, that's VoiceOver for sure.

If you can successfully unlock the phone, VoiceOver works fine. I've seen some 
bugs on occasion. Then try doing a three-finger double tap. That toggles speech.

If that does not work, unlock the phone, flick your finger right and quickly 
use the volume up button. My iThingy has sometimes turned down the VoiceOver 
volume while the sounds are fine. Keep doing this. You want VoiceOver to speak, 
even though you cannot hear it.
