Yes, Adobe is a subscription model.  There’s both PDFPen and PDFPen Pro.  PEFPen is not as feature full, but has the OCR, is very accessible and a really good package.  No, neither are free, but the Pro version is more expensive and probably has more features than you might need.  There are likely other packages out there to do the job, but this package has worked well for me.
There's supposed to be a new, redesigned version of ABBYY FineReader for Mac as well. I don't know whether the new user interface is accessible with VoiceOver, however.
