I think Monterey is the first OS on Mac to offer this feature directly.

First, in VoiceOver Utility, under the VoiceOver Recognition category, make sure that Image Recognition is enabled. You can download extra languages as well if any of your images require translation or to be spoken in another language. The first time I pressed VO-shift-l, VO said “Preview, Not Responding”, then said “No Description Available”, Thus I said “Hmph”.
