That being said, one advantage of the mouse cursor following the VoiceOver 
cursor, is that some voice over commands like 
vo-shift-M contextual menu 
Vo-space activate item 
do not seem to work as consistently when the mouse is not somewhere within the 
current VO cursor bounds.
That being said, the VO-shift-command-space (key seems to first move the mouse 
cursor to the bounds  and then do a mouse down or mouse up. The VO-Space key 
issues probably depend on the role/type of object currently in the VO-cursor.
