Could you be specific about what sounds are coming through the Mac’s speakers even when headphones are plugged into the headphone jack on the Mac. When headphones are plugged in, they should become the default output device. If you can identify what sound you hear coming from the Mac’s speakers, you should be able to set the output for that source to the default output, and not a specific device, such as the Mac’s speaker. If you are hearing VoiceOver through the Mac’s speakers, you can set the output for VoiceOver in VO Utility > Sounds, so that VO output is sent to the system default, which should be the headphones.
