Hello Carolyn, VoiceOver is already integrated into the OS and all the standard Mac commands work while VoiceOver is active.
I don't think sighted users would appreciate VoiceOver being active all the time. Many people turn VO on by accident and get very annoyed with it. If all VO commands were available at all times, people would use the commands unintentionally even more often. I tend to use standard Mac keyboard shortcuts whenever they're available. I don't think I've ever used the VO commands to go to the Dock or to the Desktop.
