Yeah. It seems to happen when you type or just press keys too fast. For example, Voiceover will just stop talking sometimes when I'm arrowing through files and folders in the finder. Then I have to press Command F5 4 times quickly to bring speech back. same thing occurs at least 2 or 3 times a day while typing.
