Hi, Hotspots are persistent through reboots. I'm not sure whether this was true in the earliest versions of the OS when they were first introduced in Leopard, though. They do have to be someplace that VoiceOver can "see", though -- else we could set hot spots for the problematic app icons that don't show up for VoiceOver navigation in the status menu bar.
