Hi George,

What I believe is happening is that you are either accidentally or purposefully 
pressing the F8 key.  Maybe you are intending to go to the VO Utility or maybe 
simply unintentional.  The F7, F8 and F9 keys are Media keys, (Previous, 
Play/Pause and Next) and if you press the F8 key, iTunes will attempt to open 
as you are telling your Mac to "Play" when you press that key.

Depending on your Keyboard settings, you may have to press the FN key along 
with F8 to access the VO Utility.  To get rid of the iTunes Library creation 
dialog, you should be able to either press ESC or cmd-period to Cancel it.  If 
it continues being stuck, you may need to Force Quit iTunes by pressing 
cmd-option-ESC.  That dialog appears if you do not have an iTunes Library 
location yet or if you press the Option key while iTunes is starting up.  Often 
if you hold down your VO keys while doing VO things, iTunes, and other apps, 
notice the Option key is down and ask the Library creation question.
