Hello Joe, As soon as you make a change in the VoiceOver Utility, it is taken into account. There's no need for an OK button.
Once you get your VO settings exactly as you want them, it's a good idea to save them either for use on another Mac, or in case your Mac suffers a major crash. You save your VO prefs in VO Utility by pressing Command-Shift-e. A dialogue box will appear in which you can name the prefs file and select where to put it. You can also choose whether to save all your VO prefs or just selected ones such as Keyboard Commander Keys, Braille Input keys or whatever.
