I'm going through this process right now. After signing up with the web site, 
the first thing you want to do is to email [email protected] and 
ask them to enable the accessible reader on your account.

Once the accessible reader is enabled, when you access a book, you will be able 
to see the text. However, there are additional optimizations that can be 
performed to make it easier to navigate with a screen reader. This includes 
descriptions of images in the book, as well as better tagging of paragraphs, 
headings, lists and tables. They say that 80% of CourseSmart titles are already 
tagged. My book was not. If you read a book and it just seems to be raw, 
unorganized text, write to the same email address and provide the title and 
author of the book so that it can be submitted for tagging. Unfortunately, this 
takes a few weeks, but you can still read the book as it is until they are 
finished with it. It is definitely usable even in an untagged form, it's just 
much more pleasant once the work is done.
