Hi,

with voiceover, you can bring up context menus with VO shift M.  In certain 
instances, you need to be interacting with the object to bring up the context 
menu for it.  For example, you need to be interacting with the media table to 
bring up the context menu for a song in iTunes.  to do a left mouse click with 
Voiceover, press VO shift space bar.  There will be some times where the 
voiceover commands will not work.  If you have a Apple mouse or, trackpad, you 
do a right mouse click by pressing control and clicking the trackpad or mouse.
