Strange. I guess you have bin in system preferences, sounds and sound output, 
set your output to your usb headset? There is also the option to route voice 
over to the internal speakers in voice over utility somewhere. You will 
probably get more helping answer soon, just strange.

As a last workaround you could do a reinstallation of OSX and preferably a 
clean install and install alll apps and stuff again. Then do a backup of a good 
time zone i.e. when all works well. Then, the next time you get into serious 
anoying problems, you only need to do a backup. In 20 minutes you are good to 
go again. I can speak for myself. It is very very nice to have a good backup. 
Absolutely worth the hard work to get the good work back in 20 minutes if i run 
into problems.
