As comments can be added by the public and they can add images via tinymce it
makes sense not to give them the 'attached image' option (like exists in
blogpost edit).
So to get same functionality working for comments tinymce insert image there
would need to be radical changes:
1) have the adding of an image use the file chooser/ uploader
2) restrict the ability to choose image in tinymce restricted to logged in users
3) make sure the changes above don't break how leap2a imports/exports currently
work with attachments on comments,

Title:
Insert Image icon in TinyMCE differs on some pages

Status in Mahara ePortfolio:
  Confirmed

Bug description:
The TinyMCE insert image dialogue differs on Blog Post to other
locations (e.g. place feedback). IMO this is a usability bug as users
will expect it to be the same everywhere (I did!).

Since feedback can include uploaded files, I'd favour use of the same
image dialogue as the blog post in the feedback section to allow
easier inclusion of inline images - perhaps there's a pedagogical
reason for not doing so though?
