Cookie Consents "Additional site modifications" without panels

Bug description:
Mahara 15.10+
The 2nd page of Cookie Consent (you need to enable Cookie Consent and
click "Save changes" in order to see /admin/site/cookieconsent2.php )
is missing some panels. See
http://manual.mahara.org/en/15.04/_images/cookie_consent_additional.png
of what it should look like. Right now, the example and "It didn't
work" are shown in full, but "Example" and "It didn't work" are still
linked. These should be panels in their individual sections.
