I have seen this problem in Linux version as well and exists still in current
CVS builds. 

It has somehow to do with the fact that after M has errors with a given server,
sometimes it decides the server is faulty and instead of asking the usual
"Server faulty do you want to retry" type question, it just ignores the user /
server instead. It is not constant but somewhat intermittent. 

I do not know exactly what conditions cause it to go into ignore mode but as you
say, restarting does cure it.
