Could also be something as simple as two memberships- [email protected] and [email protected]. I've had that happen to me, both as a user and as a list admin.
(One case of that- the mail host sent as "[email protected]" but saltmine didn't directly receive mail; incoming had to go to "[email protected]". I had two sub's to the same list, one set as nomail.)
