I have reviewed project settings, but there is no setting related to permissions to browse and upload. It has always worked before.

Note the file was only 41k in size, so it's not like it breaks the size rule for uploaded files.

When a file is uploaded by the web server, it is stored in a temporary location. For some reason, Mantis can't read the file after it is uploaded. For most servers, the upload directory must be made explicitly readable by all.
