Terry, i have uploaded a simple capture tool to the directionsmag site (qik_capture.zip), this tool includes the source code so you can see how to test for user input and apply attribute info as suggested by Mats. This tool may also do what you need in its own right as you can directly customise it to capture points (as well as lines and polys) with preset graphical and table attributes.
