It works with the WEB LOG parameter you described so there is probably a specific reason on your system causing trouble. Have you tried putting an empty file 'mapserver.log' in 'd:/temp' before starting your app? Have you checked the Windows event log files? Is the disk 'd:' a 'real disk' disk and not a mapped one? What file system do you use on 'd:'? A lot of questions and not much of a clue, I know, but maybe it will help...
