I took the first screenshot at the HapMap web site. If you go to HapMart (http://hapmart.hapmap.org/BioMart/martview), select the rel23a schema, choose the "HapMap Population -- CEPH(...)" dataset, and then in the Genotype attribute page, select the "CEU" attribute, you will see the long header in the output.
