I was just talking to another user about this and I think its hitting
you to:

        - Are you using IPAUTOFW?

I've found that IPAUTOFW, even just compiled into the kernel, will 
mess with things after a peroid of time.  If you need this kind
of functionality.. use Steven Clarke's IPPORTFW.  Its a MUCH
better solution and is maintained!
