We've tried setting up a proxy on the server to forward the request from the
old url rather than redirect, but have not had any success on this yet. But I
want to double-check that the form is set up correctly. Can you please let me
know any specifics from the code that posts to the form? What is the url it is
using? What is the id/name of the field it is posting the data to?
