I have figured out it was either an insert or update statement that
crashed the DB. I will respond again when I have the details of the
transaction.

my first guess would be that you've reached a system imposed limit of
memory/semaphores. Can you specify how the DB is configured (DATA_CACHE,
memory of the box etc.)
