Are you using the new "Debug" button in Icarus? It should all just work.
Can you confirm that the Visual Studio debugger has attached to the "Gallio.Host.exe" process? What happens if you hit the Pause button? Out of curiosity, if you previously configured Icarus to be IsolatedAppDomain, try switching it back to IsolatedProcess. It should work either way but there could be a bug. I can run the tests with Echo using the /debug flag and the breakpoints work in Visual Studio.
