Could be an issue with a mismatched app version number.

If you have ever updated or modified an app package CM will modify the apps
revision number.  But then the revision number in the task sequence no
longer matches the applications revision number and the TS process is not
intelligent enough to look for the updated revision number. There is no
easy way to update the revision numbers stored in the task sequence.  To
fix this in the TS delete the app from the TS, close the TS and save the
changes.  Then re-open the TS and add the apps back to the TS.  Applying
the changes and re-adding without closing the TS does not work.
