I found a potential solution which I'm still evaluating.  An "experimental" 
extension called CatBoxAtTop exists and does exactly what I want, places all 
specified categories, wherever they are in the page, at the top of the page in 
the upper right corner in a table.  The categories still are visible at the 
bottom as usual, this is just an extra table of the categories shown at the 
top.  Quite nice.  Works with Monobook under MW 1.15.1 but have not tested with 
other skins.

Nevio's suggestion is good for having the category name & link appear
where you want it, but it *won't* add the article to the category.

The "classic" skin puts the category names near the top right corner.
Have the user try this & other skins.
