Whether you run updates from the command-line or using the installer, you do first need to replace the old MediaWiki files with the new ones. If you run the updater/installer for the same version you're already running, it'll just do a run over the database and end up not adding anything new. (Yes it's not as convenient as the auto-update systems in say modern WordPress.
