Instead of building standard care plans, try building problems(diagnosis) 
with 1 "generic goal" and 1 specific intervention attached.  Trigger 
problems with query links and the end user will build a customized plan of 
care as they go through the assessment(s).

Ex.
Assessment query: "Pain now or in the recent past" Y/N  (Yes, triggers)
              Problem: Pain
                   Goal: "Minimize/control pain"
        Intervention: Pain, Assess/Manage
