Iatric did a report for us that pulls home meds from the admission
assessment and current meds (while in the hospital).  It's in a format that
has a check box "yes" or "no" for the physician to order which meds the pt
is to be discharged with. It also has a section that allows the doc to add
any new meds that he wants to include. It pulls everything by trade name so
ECFs really like it.

I'm just wondering what everyone is using for Medication Reconciliation at
Discharge? Especially when it comes to the discharge instructions...we would
like to be able to pull in both the home meds that the nurse enters and any
ordered meds from Pharmacy...then allow the nurse to edit whether they've
been discontinued or not at discharge....does anyone do this? Is it
possible?  If not what is your hospital's process. We are a Magic 5.4
hospital.
