If you are saying there are no diagnosis at all, no interventions at
all, on this patient - you could make that intervention an assessment,
and allow them to document it there.

However, if you have other documentation already on the patient, you can
find an appropriate place on a routine assessment that asks about the
need to add restraint documentation.  Then you use the query link
dictionary in NUR to prompt staff to add that appropriate problem
(diagnosis), outcome (goal) and intervention.

 I did not request it properly- I have no diagnosis on the care plan .
So
when the intervention is added I wanted to ADD a diagnosis/problem to
the
care plan. If I had the problem all ready on the care plan, I would be
ok.
Right now, I can only see that the nurse goes out to the care plan and
puts
the problem on that has the intervetion restraint on it. I am trying to
do
it the opposite way!

You can use the "query link" to add an intervention to the careplan (as
an "additional intervention") but you can't make it go to a specific
diagnosis:

1)   Go to the intervention dictionary
        a)      On page 1, add a OE Category and OE Mnemonic.
        b)      On page 2, answer the two "Query Links" queries
appropriately.
2)   Go to the query link dictionary.  For the query/response, add a
link to the Category/Procedure you created in step 1.

When the query link is invoked, the user will be prompted to add an
"order" for the intervention.

You can trigger a problem from the PI Screen with that intervention
attached to the Problem, but you can't trigger just an intervention by
itself. I wish you could but you can't.

Is there a way to trigger an intervention from Process Interventions to
the Care Plan? I know we can move an intervention to a problem in
process interventions by using  Move Intervention on the verb strip  but
we wondered if there is a quicker, default way to do it.
