Are there any facilities out there that have found a solution for the national standard regarding Nursing documentation of first dose administering of meds? Current policies and procedures would be great to get and even exactly how the process works from the Pharmacy to Nursing. We are trying to come up with a solution to this but are running into problems with the fact that depending on the med administered there could be a time frame of 4 hours before there could be adverse reactions all the way up to 12 hours.
