Because of the item you mentioned ... that the computer system will track which nurses added nursing diagnoses to the care plan ... we have a policy that only RN's do the admission assessment and daily risk assessment, which evaluates oral status, fall risk, braden score ... all of which could trigger a nursing diagnosis by way of query links. LPN's assist in the admission process by data collection ... filling in our 4 page paper nursing history and physical. An RN then reviews the form, signs it, and uses the info there to complete the computer admission assessment.

We are implementing NUR in late October.According to hospital policy, LPN's cannot create the initial nursing care plan (problem list). In the "paper world", the LPN does the first couple pages of the inpt database and an RN completes the "physical assessment" portion and creates the initial problem list.Any creative ways around this in the "automated world"..We have created query links to add problems to the plan of care, however if the LPN answers "Yes", her/his initials are associated with initiating that problem.Advice is greatly appreciated.Cheryl Toomey MHA, RN BCClinical Informatics SpecialistMilford Regional Medical CenterMilford, MA 01757vm (508) 422-2449[EMAIL PROTECTED]
