We had a similar need in one of our critical pathway orders .... policy is to switch from IV antibiotic to po when patient meets 3 criteria (afebrile, good po intake, and wbc count down).  No MD order needed since it was in a policy.
 
We set up a CDS for nurses to address these 3 questions, and if all were "Y", then via query link, we had an order automatically print off to PHA.  We set up a CONS type of order, and a procedure specific to this need.  PHA would then come to the floor and write the order.
 
So you could do something similar ... make a CDS that asks, "Does patient need pneumococcal vaccine?"  And if "Y", set up a query link to print of an order to PHA.

We are currently using a paper form for assessing patient need for pneumonia and flu vaccinations.  Would like to document in Meditech.  We do not use POM or eMAR at this time.  How can I have the nurse document the assessment in MT and automatically send this as an order to pharmacy?  Our policy is such that if the patient meets the criteria and there are no contraindications the physicians order is not needed.
