To Lr's, What is your communication process between Nursing and other disciplines to relay pertinent patient observations/information (ie. Speech Therapist finds that the patient is at high risk for aspiration, wants nursing to keep a close watch on the patient while eating)? Do they do this thru Patient Notes?
