Look at the Audit Trail - if you see an entry with the user "interface",
that is a change that happened on the RAD side.

Did you say that the Service Date is different in OE from in Rad?  Is the
order still active in OE, or does it show as "CNC" cancelled?  

In the dim recesses of my mind I think I remember this comment (Order # xxxx
replaced by order # xxxx).  I think the test was changed on the lab/rad
side, but there wasn't a corresponding test on the OE side.

Let me know what you find out.

We are a 5.4 site and I have never seen this happen. I really sounds like
someone may have changed the order and you are aware of the excellent
tracking tools in radiology that provide no information. I would also check
to see if the OE person is verified if your site has OE users as a verified
user. The service date should not change unless there are two orders, one
generated by OE (original order), and the other generated by Rad since they
never received the first order. 
I am not sure if this helps. Please post to the L once a solution is found.

Has this ever happened to anyone? This is the 2nd time that this problem
occurred. Nursing placed an order for ultrasound. Based on the OE Audit
trail, the service date and time is the same as the order date and time but
when you look on the RAD Exam Profile, it shows a service date of the
following day. On the Display Order routine in OE, it also has a comment
saying that the Order # ------ was replaced by Order # -------. 
I checked all the background jobs (OE, RAD and OPS) and I don't see anything
conclusive. I'm not sure at this point whether the Service Date was manually
changed by a RAD user or by the system. Where do I track this activity? The
first time this happened, MEDITECH didn't find anything either. They think
that the requisition may have been misplaced. We left it at that since it
can be a big possibility. Now that it happened again (on the same category
but different procedure), I doubt that the requisition was misplaced because
the nature of the problem is exactly the same as the previous one. Any
thoughts on why this is happening? Thank you very much for your input and
