We have a fairly short list of allergy & interaction override comments Allergy Responses H - History of previous use of this or similar med. (Typically this would be determined from review of meds in PCI or the H&P) C - Caution statement in order and on MAR. (Requires placing information into the Order Comments advising the nurse about the nature of the problem and what to watch for. Used when it is reasonable for the patient to have a trial course of medication.) CI -See Clinical Intervention (Used to document resolution of all other situations, especially where MD contact occurred) Interaction Responses AJ - Adjust medication schedules to prevent interaction. PJ - In the pharmacist's professional judgment, no action is needed. CI - As above. Use to document pharmacy monitoring of potential interactions and all MD contacts. C (As above) H (As above. Often used if patient has been using interacting meds at home.)
