Does anyone have a report that outputs in a flat file one record for each of the patient's operations? The information that I need for each record is acct number, oper.date, oper.proc, oper.surg, oper.cpt, oper.cpt.mod[1] and oper.cpt.mod[2]. I can get the first 3 fields, but the surgeon only appears with the first procedure for the oper.seq.no. I haven't been able to pull in the cpt & modifiers at all.
