Good morning, Herb.  We split the tablets in the Pharmacy and they are
loaded in Meditech as a separate drug entry and into Pyxis as a separate
drug entry.  We use the NDC number of the original tablet with "H", or
"Q" appended to the end for half or quarter.  Meditech seems to handle
this well.  We are MAGIC.

Wondering if hospitals could comment on how they handle frequently used
1/2 tabs.  Is a separate pharmacy mnemonic created?  Or, do you use the
manufacture's size and then enter the correct dose.

Also, what is loaded in the dispensing machines?  The 1/2 tabs, or the
full tab and nurses break the tab?

Examples: 
Coumadin 0.5 MG  (Is an entry created for the 1/2 MG (1/2 tablet) and
then loaded into Pyxis/Omni), or is the 1 MG mnemonic used with a dose
of 0.5 and the nurse breaks the tablet.

Other examples/meds could be
Metoprolol 25 mg (before the 25s were available) HCTZ 12.5 mg Amiodarone
100 mg
