Terry: We are using the POC routine. We started using it in 2000, even before most of the nurses were doing on-line documentation. We currently have 232 diagnoses/problems built and counting. They are fairly specific as they were built off the paper diagnoses we used in the past (we used to pull pre-printed diagnoses and, after modifying them, stapled them together to made up a care plan) and they also include all the therapies. The reason we opted to make them fairly specific was a cost-effective choice: the less time it takes to build a care plan, the more time the nurses will have with the patient. The POC routine isn't very user friendly, even with NUI, the nurses have trouble remembering how to edit text, change priorities and even get to the goals. Teaching this routine takes up the bulk of the NUR training time for both RNs and therapists. I would love to see Meditech rework this routine so that it would be easier for the nurses to make changes. Still, I have to say our nurses do a good job in individualizing the POC. The therapies have a much easier time with the goals because they have always done goal-driven documentation whereas nurses tend to do intervention-driven documentation. They usually only take a couple of days in learning how to get to the goals; whereas, it takes a good six weeks before the nurses are comfortable with getting to them. (This is because the therapies build specific goals, free text, on every patient but the nurses only need to modify goals on some patients). We would like to know how other hospitals are documenting a Plan of Care. Are you using the Plan of Care routine? If so, how specific are your Diagnosis (and how many do you have)?
