We have been using the POC routine for over 8 years. We have also been in the process of revamping all of POC's for over a year now. We started out with everything that could ever be needed on the POC and found out that was not the best idea. Our redesign also includes simplifying the diagnosis and goals to make them more user friendly. Nurses can make them more specific when needed.
