We have been using the POC routine for over 8 years.   We have also been
in the process of revamping all of POC's for over a year now.  We
started out with everything that could ever be needed on the POC and
found out that was not the best idea.  Our redesign also includes
simplifying the diagnosis and goals to make them more user friendly. 
Nurses can make them more specific when needed.  The nurses document the
Goals at least every 72 hours and at discharge.

Good Morning,

One of our hospitals is looking to re-design the Plan of Care.  We
would
like to know how other hospitals are documenting a Plan of Care.  Are
you using the Plan of Care routine? If so, how specific are your
Diagnosis (and how many do you have)?  If you do not use the Plan of
Care Routine, how do you document and define Goals/Outcomes?
