Title: Blood Band Transfusion Monthly Statistics

I have been asked by the Blood Bank Supervisor to create a monthly report that:
captures patients transfused on a monthly period that would print them listing all blood products transfused for that given period of time and also capture their pre and post hemoglobin/hematocrit, pre and post INR, pre and post platelet count and admission diagnosis.

I have poked around Meditech, and there does not seem to be very many customs for Lab.  If any one is willing to share a report that may meet at least some of the requests, I would greatly appreciate it.
