Yes, the user during orientation signs many forms, including the computer access & password confidentiality agreement. However, we set up the logon information (usually) before they arrive for orientation, and give them that information at the time they sign the form. This leads to some inconsistency of timing; we have some managers who are very good about sending the request in time, others who don't get it until after the new employee has started. We use an Outlook form, so it's online, not paper. This has the advantage of being quick to fill out, and then everyone (HR, IS, other departments) are all notified simultaneously. In IS, we almost always accept the manager's request, and provide the requested access. In some cases, we may question a manager as to why a certain user needs certain access, or conversely, we may point out to the manager that the user needs certain access that's not requested on the form. But most of the time, we provide the access as requested. It works pretty well, the only issue being the managers who are perpetually late on their requests. Good luck!
