A long time ago a wonderful person gave me this val statement to get the
current price on a BAR procedure. The logic of it remains a mystery to me.

Then someone else gave me this to get the CPT-4 (or whichever alt.code.type).
Ditto on the logic

Now I need to get the actual effective date of the alt.code and I don't have a
clue how to extract it.
